I have sex about once a week. Daily 2.5-5 mg or a PRN tablet?
One INN, two calendars, one nitrate hold
Claim: daily Cialis and PRN Cialis are different drugs. Label: false. Strike the two-molecule myth. File two schedules of tadalafil.
As-needed tadalafil is the older habit. You swallow before you expect sex, usually 10 mg, and you use a window that can stretch toward 36 hours so you are not watching a kitchen clock. Daily tadalafil flips the logic. You swallow 2.5 mg or 5 mg every day, levels stay quiet and steady, and the tablet is already there if the evening turns. Neither calendar is a better molecule. They are two ways to wear the same linger.
The linger is why the choice exists. A four-hour drug cannot honestly offer a breakfast carton. Tadalafil can. The enzyme detail is in the tadalafil reference. Before any preference talk: nitrates are closed on both calendars. A daily 2.5 mg chip does not buy a nitroglycerin pass. The hold is longer than a workday because the half-life is.
A quiet week belongs to PRN 10-20 mg
If sex is roughly weekly or less, a daily bottle is a month of swallows for a handful of evenings. That is extra drug, extra cost, and no prostate job unless you also have one. Start as-needed at 10 mg. Drop to 5 mg if the effect is plenty and the head or back complains. Rise to 20 mg if 10 mg is a near-miss and you tolerate it.
Claim: PRN tadalafil still forces Viagra-style timing. Label: leftover from the wrong INN. Strike: a morning swallow can still cover that night and the next. You are not parking a 60-minute alarm. You are picking a day. Men who can loosely see an evening coming - travel, a planned night, a weekend - waste less tablet this way.
The friction that remains is a small act of forethought. For some couples that is nothing. For others it is the whole problem: the pause to find the blister kills the mood. That complaint is how I end up writing a daily 2.5 or 5 mg script even when the arithmetic of frequency looks PRN-friendly.
A busy or unplanned week belongs to 2.5 or 5 mg daily
Claim: daily tadalafil is for men who failed the strong tablet. Label: backwards. Strike. File it as an anti-planning habit.
Take 2.5 mg or 5 mg once a day, same clock if you can. After a few days you are at steady state. Nothing to swallow 'in time.' This is the carton I reach for when a couple says the planning itself is the injury, or when sex is frequent enough that a daily chip is not a waste. Dose-for-dose it is a quieter peak than 20 mg PRN. That is the point, not a defect.
It is a poor fit if evenings are rare. You would be dosing thirty days to cover three. It also asks for a daily habit, which some men will not keep. In the ED trials, 5 mg is the better-measured daily strength. 2.5 mg is the labelled quieter start. I do not treat them as equals, and I do not call 5 mg daily a failed 20 mg.
Give the breakfast carton a few weeks before you declare it weak. One sleepy Sunday is not Rajfer's 24-week file. If after a fair run the erection is still a near-miss, then a PRN 10 or 20 mg trial is a reasonable switch, not a moral failure.
Only the daily 5 mg tablet can hold a prostate score
Here is the split that settles men in their sixties. Lower-urinary-tract symptoms from an enlarged prostate - weak stream, strain, night trips - move on the IPSS with tadalafil 5 mg daily. Roehrborn's ladder is why 5 mg, not 20 mg, is the labelled urinary dose. As-needed Cialis cannot do that job. A chronic symptom needs a drug that is present on Tuesday morning, not only on Saturday night.
So a man with both an enlarged prostate and erectile dysfunction gets one 5 mg daily tablet and two scores. That is often the whole decision. If the stream is the louder complaint and flow is truly obstructed, an alpha-blocker or a 5-alpha-reductase inhibitor may still do more for the pipe. Sometimes we combine, carefully, because two blood-pressure-lowering ideas can stack. The trial read of the overlap is in the endpoint review.
Do not file 'I have a prostate' as an automatic 20 mg PRN. Wrong carton for that claim.
Peak is the PRN boast. Presence is the daily one.
| Split question | Daily 2.5 or 5 mg | PRN 10-20 mg |
|---|---|---|
| Swallow | Same clock, every day | Before a seen evening |
| Week that fits | Frequent or unplanned | Rare, loosely planned |
| Planning | None after steady state | A day, not a minute |
| Peak | Lower, flatter | Higher at 20 mg |
| IPSS / BPH | Yes, at 5 mg | No |
| Tablets per month | 28-31 | A handful |
| Nitrates | Closed | Closed |
A 20 mg as-needed swallow posts a higher peak than a 5 mg daily swallow. Men who chase the single best night, and who can live with a little planning, will feel that. Men who want the drug already in the room will not miss the peak as much as they expect. Rajfer did not show that 5 mg daily matches 20 mg PRN on a one-night ceiling. He showed that 5 mg daily beats placebo on IIEF-EF and SEP3 without a clock.
Side-effect texture follows the same split. Daily 2.5 or 5 mg tends to be a lower, flatter complaint - stuffy nose, some dyspepsia, occasional ache. PRN 20 mg is more likely to announce itself the next day in the lumbar muscles. Neither pattern is a reason to ignore a faint or a chest pain. Both patterns are a reason to adjust the carton rather than to stack cartons.
Sildenafil and vardenafil do not offer this split. Their half-lives are a workday. If you need a breakfast habit, you are on tadalafil or you are on a different conversation. Their medicine pages - sildenafil and vardenafil - are timing and food files, not daily-carton files.
Generic math killed the luxury argument, not the tablet count
Claim: daily Cialis is always the expensive choice. Label: dated. Strike the brand-era fear. File tablet count.
When only brand Cialis existed, a pill every morning was a luxury bill. As-needed won on price for anyone who was not having sex most days. Generic tadalafil flattened that cliff. Daily still uses more tablets. It is usually a bit more per month if evenings are rare. It is no longer the reason to refuse a breakfast habit that would otherwise fit.
Think per-use versus per-month. Rare sex: buy the evenings. Frequent sex: the per-occasion cost of 2.5 or 5 mg daily can look sane, and you keep spontaneity plus any prostate work. Prices move by pharmacy and plan. Check a local card instead of a 2012 memory. The FDA's consumer pages at the FDA will not quote your cash price. Your pharmacist will.
You can switch cartons. You cannot stack them.
Claim: once you pick daily or PRN you are stuck. Label: false. Strike the life-sentence. File a supervised swap.
It is the same molecule. Men start as-needed, hate the pause at the blister, and move to 5 mg daily. Others start daily, notice they needed the tablet four times a month, and go back to 10 mg PRN. Both are ordinary. What is not ordinary is swallowing the breakfast chip and a 20 mg 'booster' the same night because the evening felt important. That is a stack, not a schedule.
Change the carton with the person who wrote the first one. Give the new habit a few weeks. A single occasion lies. If a strong CYP3A4 inhibitor or an alpha-blocker is already on the list, the start is quieter and the timing is more careful on either calendar. Those details live on the tadalafil reference, not as a second monograph here.
The hold does not care which calendar you picked
Nitrates are closed. Riociguat is closed. Recreational nitrites are closed. Daily 2.5 mg does not open them. PRN 5 mg does not open them. If emergency chest-pain treatment is ever in the room, the team needs the tadalafil fact, because the linger outlasts the shift.
Alpha-blockers for pressure or for the prostate can add to the blood-pressure drop. That is a start-low and separate-the-clocks problem, not an automatic ban, and it is easy to confuse with the nitrate ban. The first is caution. The second is a wall. Bring the full list. Let someone who has examined you sort the wall from the caution.
Mayo Clinic's overview at Mayo Clinic is a fair plain-language companion. It is not a substitute for a nitrate check.
File: count the week, then pick the carton
If evenings are rare and planning is fine, order the PRN carton and start at 10 mg. If evenings are frequent or the pause to plan is the injury, order daily 2.5 or 5 mg and expect 5 mg to be the better-measured habit. If an enlarged prostate is in the room, the daily 5 mg tablet is the only Cialis calendar that holds an IPSS. If a nitrate is in the room, neither calendar is yours.
That is the split. It is not a strength contest and not a reprint of the medicine page. Change nothing about a dose until the person who knows your list has heard the week you actually live.

Most mail about tadalafil is a calendar argument dressed as a strength argument. I split the week.
For once a week I usually write PRN. Thirty daily chips to cover four evenings is extra drug for little extra readiness, unless the planning itself is the fight or you also need a prostate score. Tadalafil's window makes as-needed easy: a tablet earlier in the day still covers the night and often the next. Start at 10 mg. Move to 5 or 20 on effect and ache. If your partner hates the pause at the blister, that single complaint can flip me to 5 mg daily even on a quiet week.
He is 66, up twice a night, and ED is part of the same year. Why does everyone say daily 5 mg?
Because that is the only Cialis calendar that can hold both scores. Five milligrams daily moves IIEF-EF and IPSS. A 20 mg Saturday swallow cannot treat Tuesday's stream. Roehrborn is why we do not chase 20 mg daily for urine - the extra IPSS is tiny and the ache is not. If the pipe is truly obstructed, I may still want an alpha-blocker or a reductase inhibitor, and I watch the pressure if we combine. As a single tidy tablet for both complaints, daily 5 mg is the file. MedlinePlus sketches the prostate picture at MedlinePlus.
I do not want a weaker erection. Is daily 5 mg a step down from 20 mg?
It is a lower peak. It is not a failed 20 mg. Daily 5 mg keeps a steady level that, for most men in Rajfer's kind of file, is enough whenever the evening happens. You trade the single-night ceiling for no clock. If you have given daily a few honest weeks and the erection is still a near-miss, that is a real reason to go back to PRN 10 or 20 mg. If you judged it on one tired night, you did not run the study. Try the habit before you mourn the peak.
Now that tadalafil is generic, does cost still pick the carton?
Less than it did on brand Cialis, which is why I stopped leading with money. Daily still uses more tablets, so a quiet week is still cheaper as-needed. A busy week can make 2.5 or 5 mg daily look reasonable per occasion, and you keep the unplanned evening and any IPSS work. Check the pharmacy you actually use. A 2012 'daily is a luxury' memory is a stale claim. Label it dated and file today's cash price.
Can I keep 5 mg daily and take 20 mg when I travel?
That is a stack, not a split. Same INN, two cartons, one night - you are inventing a dose the label did not study as a hobby. You may switch calendars with the person who wrote the first script. You may not layer them because a hotel evening feels important. If travel is rare, stay PRN and take the 10 or 20 mg tablet on the trip. If travel is your real life, stay daily and do not 'boost.' Ask before you improvise.
He takes an alpha-blocker for the prostate. Does that pick daily or PRN?
It picks caution, not a carton. Alpha-blockers and tadalafil can add to a blood-pressure drop, so we start low and separate the clocks. That is true on 2.5 mg daily and on 10 mg PRN. The question that actually closes the class is a nitrate, which is a wall, not a timing trick. Bring the full list. If the alpha-blocker is already doing the stream work, daily 5 mg may be redundant for urine and still useful for ED - or the reverse. That is a person-level call, not a blog rule.
Is taking 2.5 or 5 mg every day for years a reckless idea?
For the right person it is a labelled, studied habit, not an experiment. Daily 2.5 and 5 mg were built for continuous use in ED, and 5 mg for urinary symptoms. Extensions out to a couple of years did not unmask a new toxicology. Headache, flush, dyspepsia, a stuffy nose, sometimes the next-day ache - those are the familiar complaints, usually mild. Reckless is a nitrate, a hidden heart symptom, or a stacked booster. Inside the hold, daily is a legitimate calendar. The label's long-term language is at the FDA.
We tried sildenafil and the meal ruined it. Does that push us to daily Cialis?
It pushes you toward tadalafil, not automatically toward the breakfast carton. Food does not blunt tadalafil the way a greasy plate blunts sildenafil. You could still do PRN 10 mg and keep the long window. Daily 2.5 or 5 mg is the extra step if planning, not pizza, is the remaining injury, or if a prostate score is in the room. Read the meal problem as a reason to leave sildenafil's clock, then split tadalafil's two calendars the usual way - count the week.
Where should I read if I want the dates or the IIEF points, not this split?
Dates and cartons: the Bothell development file. IIEF, SEP3, IPSS, PHIRST: the trial-number review. Enzyme, interactions, the full hold list: the tadalafil reference. This page only answers which calendar to swallow once those files are already clean. Do not change a dose from a library page. Take the week you actually live to the person who can see your list.
